    221 Essential Tremor & High Frequency Ultrasound – Smith May 17, 2018

    Essential Tremor Responds to High-Frequency Ultrasound

    Steve Smith(his real name) owns a small technology reseller and has been dealing with Essential Tremor - ET - a widespread but little-understood condition, for over ten years. It impacts his quality of life daily, as he struggles to drink a cup of coffee, hold a screwdriver and type.

    Last year, his symptoms worsened, and his golf play deteriorated to the point where he could no longer compete.

    As you will hear in his report he couldn't hold a club.

    High-Frequency Focused Ultrasound - For Essential Tremor

    Then one year ago, in March, Smith underwent a new non-invasive procedure approved by the FDA just last year. Suddenly, his hand wasn't shaking any longer.

    Of Note: More than eight million people suffer from Essential Tremor and other movement disorders, but many remain undiagnosed and ineffectively treated. This avid golfer loves to share his story with others - it's truly inspirational.

    Also called High Intensity Focused Ultrasound - HIFU - Steve was treated in New York by Dr. Michael Kaplitt.

    Note: We have Dr. Kaplitt scheduled as a CBJ Guest quite soon, and his interview will come through our publishing schedule about a month from this CBJ/221 publication date. See more info on Dr. Kaplitt below, and stay tuned for that interesting conversation. One question I plan to review with him is the subject of the application of HIFU for Parkinson's.

    About Essential Tremor

    In addition, ET is hereditary, and in Steve's family was passed down by his mother. His brother and sister both suffer from ET. Essential Tremor started to show when he reached the age of 50. He's now 62.

    Essential Tremor occurs while doing normal things, for example, holding a cup of coffee, holding a fork or spoon to eat, etc. In particular, Essential Tremor became obvious in his golfing, putting and while at rest with the club in his hand. Smith reports, "It's very difficult to putt when your hands shake."

    His Doctor, Dr. Michael Kaplitt, MD, Ph.D. Reports on HIFU

    "Yesterday I became the first doctor in New York to perform an exciting new procedure on a patient with essential tremor, using high-intensity focused ultrasound to target the source of the tremors. I’m happy to report that the patient did extremely well, and the surgical team watched throughout the procedure as his tremor diminished visibly before our eyes.

    Focused ultrasound takes advantage of a technology that has been successfully used to treat uterine fibroids and breast and prostate cancer, but of course, any procedure involving the brain is far more complicated than treating any other part of the body. Ultrasound waves have been shown to be benign in the body, but they normally scatter when passing through the skull.

    The new procedure relies on magnetic resonance imaging (MRI) to pinpoint the exact location in the brain responsible for the tremor, then uses MRI guidance to focus the many ultrasound waves on that location.

    In yesterday’s procedure, the patient was fitted with a helmet that held an array of 1,000 sources of ultrasound energy, each beam so small and low-energy that it passed harmlessly through healthy brain tissue on its way to the lesion that was causing the tremor. When all 1,000 beams reached that lesion together in one tightly focused treatment pulse, the combined energy destroyed the lesion — without damaging any healthy brain tissue.

    All of this was accomplished without a knife or an incision.

    My team and I mapped out the target and then monitored the procedure as MR thermometry measured the temperature at the target site; it automatically adjusted the energy emanating from each of the 1,000 sources so that precisely the right amount of energy reached only the target, without affecting any of the nearby structures or the rest of the brain. We knew that no single wave was damaging any brain tissue, but that the sum of the energy from all of the hundreds of different waves focused on the target was enough to eliminate the problematic region.

    As a result, the patient’s tremor improved within seconds of reaching the right temperature at the exact spot in the brain responsible for this problem."

    My uncomplicated point at AGS: If we aim to treat the entire Spectrum, then specific medications can help - if used correctly. The reason for so much medical treatment failure with those trying to treat ASD issues with psych medications is at once simple and complex: overt denial of underlying biomedical impediments that interfere both with meds and supplements. Consider, for example, the pervasive national denial with ubiquitous IgG issues.

    Because so much of ASD often does involve Executive Function why not put that subject on the table? At AGS I discussed the most pervasive problem in psychiatric diagnosis and treatment today: Cognition, Executive Function Diagnosis, and Medical Treatment including Biomedical Interventions. Dysregulation of Executive Function with Cognitive Desynchronization does exist beyond the labels and appearances of ADHD. Due to the metabolic challenges with ASD, one must understand the medical parameters and options to intervene as correctly and comprehensively as possible.

    I've known Dr. Woeller for many years and find him to be an exceedingly helpful clinician with remarkable translation skills regarding addressing the problematic reality of hard data provided by specific autism markers and then synchronizing it with clinical observations & treatment.     James B. Adams, Ph.D., is a President’s Professor at Arizona State University, where he directs the autism/Asperger’s research program, though he originally taught chemical and materials engineering there. Dr. Adams also holds a post at the Southwest College of Naturopathic Medicine. He is also the president of the Autism Society of Greater Phoenix, the co-chair of the Autism Research Institute’s scientific advisory committee, and has received the Autism Service Award from the Greater Phoenix chapter of the Autism Society of America.

    Background

    Autism spectrum disorders (ASD) are complex neurobiological disorders that impair social interactions and communication and lead to restricted, repetitive, and stereotyped patterns of behavior, interests, and activities. The causes of these disorders remain poorly understood, but gut microbiota, the 1013 bacteria in the human intestines, have been implicated because children with ASD often suffer gastrointestinal (GI) problems that correlate with ASD severity.

    Several previous studies have reported abnormal gut bacteria in children with ASD. The gut microbiome-ASD connection has been tested in a mouse model of ASD, where the microbiome was mechanistically linked to abnormal metabolites and behavior.

    Similarly, a study of children with ASD found that oral non-absorbable antibiotic treatment improved GI and ASD symptoms, albeit temporarily.

    Here, a small open-label clinical trial evaluated the impact of Microbiota Transfer Therapy (MTT) on gut microbiota composition and GI and ASD symptoms of 18 ASD-diagnosed children.

    Results

    MTT involved a 2-week antibiotic treatment, a bowel cleanse, and then an extended fecal microbiota transplant (FMT) using a high initial dose followed by daily and lower maintenance doses for 7–8 weeks.

    The Gastrointestinal Symptom Rating Scale revealed an approximately 80% reduction of GI symptoms at the end of treatment, including significant improvements in symptoms of constipation, diarrhea, indigestion, and abdominal pain.

    Conclusions

    This exploratory, extended-duration treatment protocol thus appears to be a promising approach to alter the gut microbiome and virome and improve GI and behavioral symptoms of ASD. Improvements in GI symptoms, ASD symptoms, and the microbiome all persisted for at least 8 weeks after treatment ended, suggesting a long-term impact.

    Similarly, clinical assessments showed that behavioral ASD symptoms improved significantly and remained improved 8 weeks after treatment ended. Bacterial and phagedeep sequencing analyses revealed successful partial engraftment of donor microbiota and beneficial changes in the gut environment. Specifically, overall bacterial diversity and the abundance of Bifidobacterium, Prevotella, and Desulfovibrio among other taxa increased following MTT, and these changes persisted after treatment stopped (followed for 8 weeks).

    212 ADHD Prisoner Education To Prevent Recidivism & Relapse – Battinelli Apr 17, 2018

    ADHD, Prisons & The Opiate Epidemic

    Beth Battinelli Ph.D. reports on her exciting prisoner experiences with prison inmates who have no idea of their brain reality, especially regarding the underlying executive function challenges that led them to poor judgment, impulsivity and breaking the rules. In this CBJ/212 Episode, she talks about a program she developed: The ADHD Project.

    Her mission for the ADHD Project is to increase ADHD awareness, understanding and recovery for inmates in corrections. It's an educational program that starts with primary brain science education for incarcerated individuals, and by using existing online resources, and it's a cost-effective program to continue upon the individual’s release - for example, CoreBrain Journal's Neuroscience Clubs.

    She also wants to either start or join inevitable conversations about paradoxical reactions to pharmaceuticals for those incarcerated. The current news about the opioid crisis directly connects to both of these topics. Forewarned and educated is likely to reduce recidivism in this vulnerable, underserved population.

    Well done, Beth - certainly a worthy project for undeniable difficulties.
