Student Focused Presents 230 Subgingival Irrigation: Does it improve periodontal therapy? Jun 03, 2020
    Show notes

It's A Tale of Two Hygienists' monthly student roundtable episode, which means Andrew and Michelle have invited a quartet of guests to educate student listeners on a chosen topic. Today's topic is subgingival irrigation, a form of hydrotherapy used to prevent further deterioration of tooth structure.

Spring Hatfield is a dental hygienist and co-founder of Dental Masterminds, LLC, which offers study clubs and CE courses for dental professionals. Spring's work focuses on the connection between oral and systemic disease. Daniel Lopez is a hygienist with Access Community Health Center in Manhattan who works with patients affected by HIV.

In this episode, our guests introduce, summarize and discuss the studies they've brought to the table, explain why ozone application needs to be "just right," and examine the bias where a journal's country of origin can unconsciously affect how seriously we take it.

    EPISODE HIGHLIGHTS:

    Interview starts: 4:03

    - Spring summarizes the findings of a study on ozone-infused waters and oils used in subgingival irrigation.

    - Explaining "the Goldilocks principle" and how ozone relates to it.

    - What is a "free radical" and why should hygienists be concerned about them?

    - Daniel discusses a study that makes the case for using 10% povidone-iodine during irrigation.

    - Why it's vital to overcome one's bias toward certain journals based on their country of origin.

    - Debating the merits of doing a pre-procedural rinse.

    QUOTES:

"We do kinda graduate thinking that only our research is relevant."

"Homecare is where it's at, period."

"That rinse with an alcohol-based mouthwash really helps decrease that blood flow."

"See what is available to you, see what's out there, for those patients you otherwise just can't help."

229 How Do We Care For Dental Implants with Melissa Obrotka, RDH May 27, 2020
    Show notes

To continue A Tale of Two Hygienists' implant series, Michelle spoke with a 20-year dental hygiene veteran who knows that maintaining dental implants requires a significantly different skill set (and set of tools) than conventional natural tooth cleaning.

Melissa Obrotka works as a hygienist for a prosthodontic practice in New Jersey while also serving as a clinical trainer for Electro Medical Systems (EMS). Called the "implant maintenance guru" by A Tale of Two Hygienists' own Michelle Strange, Melissa also teaches as a clinical adjunct professor at her own alma mater of Bergen Community College in NJ.

In this episode, Melissa breaks down both the broad strokes and the nuances of treating implants, impresses the importance of disclosure when discussing implants with your patients, and explains why the learning process is still ongoing well into one's career.

    EPISODE HIGHLIGHTS:

    Interview starts: 16:22

    - How hygiene school turned Melissa from the "worst student" into one of the most enthusiastic students.

    - Why treating implants is a "whole new ballgame" compared to treating natural teeth.

    - The reason implant dentistry demands further (and constant) learning.

    - Getting into the nitty gritty of working with implants as a dentist or hygienist.

    - Melissa lays out the short- and long-term rewards of investing in the latest technology for the best possible implant care—and what you can do and say to convince your dentist.

    - What can patients inspect from your average implant inspection?

    - Melissa discusses how the Dental Implants Uncovered Facebook page came to be and what you can get out of it.

    QUOTES:

"We see the implant patient from the surgical to the restorative."

"These people have made a giant investment in improving their oral health."

"It is expensive technology, but it's amazing technology and there's a reason why the price point is where it's at."

"They went the distance; I need to go the distance for them."

"If you don't take action on the knowledge, then what is the knowledge for?"

Episode 228: A guide to improve perio and implant case acceptance with Miranda Beeson, RDH May 20, 2020
    Show notes

Continuing A Tale of Two Hygienists' latest series, Michelle and Andrew have brought on a guest who knows firsthand that clear and empathetic communication is essential to increasing case acceptance, especially when it comes to discussing implants.

Miranda Beeson is a hygienist based in Virginia Beach who specializes in implant care coordination. Miranda is too familiar with the unfortunate way dental hygienists and other professionals can "buff out" the clinical side of their knowledge base without investing enough time in communication and business.

In this episode, Miranda impresses the importance of communication in dental hygiene work, compares and contrasts the language you should and shouldn't use when discussing implant procedures, and lays out her guide for getting off on the right foot with a patient. As well, Andrew and Michelle shine a light on a five star review one of their listeners gave the show on Apple Podcasts!

    EPISODE HIGHLIGHTS:

    Interview starts: 9:28

    - Miranda discusses her transition from hands-on clinical work to an implant care coordination role.

    - The moment Miranda realized how essential communication—especially wording— is to hygiene work.

    - The key difference between "inward" and "outward" mindsets.

    - On asking the right questions to know what temperament patient you're dealing with.

    - Miranda walks us through the steps she takes discussing implant planning, procedure and care with patients.

    - Some essential words and phrases for clearly communicating with your patients while still keeping them at ease.

    QUOTES:

"I wanted to have like a solid decade of clinical hygiene under my belt before I really transitioned out of it."

"If you just flip that mindset to make it more about them than about you, that's where it all really begins."

    "Ask more and talk less."

"Cost is a barrier that's thrown out there I think far more than it truly is the barrier."

"I'm carrying that weight as your liaison just as much as I should be."
