SHORTS: Determining Risk of Falls in Community Dwelling Older Adults: A Systematic Review and Meta-Analysis Using Posttest Probability #23YearsofDataInOneTable #GameChanger May 04, 2017

Lusardi M, Fritz S, Middleton A, et al. Determining Risk of Falls in Community Dwelling Older Adults: A Systematic Review and Meta-Analysis Using Posttest Probability. Journal of Geriatric Physical Therapy. 2010;40(1), 1-36.

"Falls and their consequences are significant concerns for older adults, caregivers, and health care providers. Identification of fall risk is crucial for appropriate referral to preventive interventions. Falls are multi-factorial; no single measure is an accurate diagnostic tool. There is limited information on which history question, self-report measure, or performance-based measure, or combination of measures, best predicts future falls."

"Purpose: First, to evaluate the predictive ability of history questions, self-report measures, and performance-based measures for assessing fall risk of community-dwelling older adults by calculating and comparing posttest probability (PoTP) values for individual test/measures. Second, to evaluate usefulness of cumulative PoTP for measures in combination."

Use the link below to calculate cumulative PoTP by inserting the highest PoTP value gathered (use table 5) into the Pre-test probability box. Then use the +LR of your follow-up question/outcome measure/self-report (also from the table) in the likelihood ratio box below it. Hit re-calculate for a new cumulative PoTP.

Don't forget to take into account negatively answered questions, or outcome measures/self-report tools outside of the cut-off scores by using the -LR to go in the opposite direction for a cumulative "decreased fall risk" score. I suggest doing this first, then going back up using +LR responses.

Showing Our Value, The Problem w. Dirty Data, & Becoming Public Health Therapists w. Ken Miller, PT, DPT, GCS, CEEAA

How do you show your value? Are you able to back up what you do? Is a data database the answer to all of our reimbursement & payment problems?

In this episode, I chat with Ken Miller about value based care and issues with dirty data. We also chat about his perspective on PT's becoming Public Health Therapists. Ken is a brilliant PT with experience in several settings. He is also an experienced educator with MedBridge & with our sponsor, Great Seminars & Books. He's also very active in the Home Health Section of the APTA.

This is a wide ranging conversation that I know you'll enjoy!

    Yes!! YOU are in Sales

I know you don't believe it. You need to though. Every interaction you have can help or hurt your ability to sell your services, brand, &/or company...even if you are an employee!

Listen in on a great convo I had with good friends at our local brewery. I was joined by:

    • Jeff Musgrave, former SRP guest, is a new Home Health PT. You may remember him from this episode.
    • Trey Beckett, my brother-in-law, is a Field Marketing Specialist for Saucony Footwear & Apparel.
    • John Thornton is the Marketing Manager for Gentiva Home Health in Lexington, KY.

This episode is jam packed with practical advice and unique perspectives that will be applicable to YOU in your work. Enjoy!

SHORTS: Knee OA in the Older Adult: What Does the Evidence Say?! #JGPT #SystematicReview #WOMAC Apr 14, 2017
    Show notes

Anwer, S., Alghadir, A., & Brismee, J. (2016). Effect of Home Exercise Program in Patients With Knee Osteoarthritis: A Systematic Review and Meta-analysis. Journal of Geriatric Physical Therapy, 39(1), January-March 2016, 38-48.

"The Osteoarthritis Research Society International recommended that non-pharmacological methods include patient education programs, weight reduction, coping strategies, and exercise programs for the management of knee osteoarthritis (OA). However, neither a systematic review nor a meta-analysis has been published regarding the effectiveness of home exercise programs for the management of knee OA."

"The purpose of this systematic review was to examine the evidence regarding the effect of home exercise programs with and without supervised clinic-based exercises in the management of knee OA."
