S2 Ep86: Will THAT Really Help Your Hot Flashes? The TRUTH About The Impact of Lifestyle and Other Interventions Jul 27, 2023
Show notes Many women are highly motivated to manage their hot flashes without hormones, drugs or sketchy herbs and spices. But before investing in a pricey cooling device or signing up for a year’s worth of unlimited yoga, it helps to know what works—and what doesn’t. 86: When Hot Flash Hell Meets Headache Hell: Migraines and Menopause Jul 20, 2023
Show notes It is beyond unfair that when someone with menstrual migraines is finally heading into menopause and no longer must worry about monthly migraines, those same women often experience an uptick in frequency of headaches due to the peri menopausal hormone roller coaster.
In this episode, Dr. Streicher discusses:
    • The relationship between hormones and migraine headaches
    • Why birth control pills are often not prescribed to women with migraines with aura
    • If it is safe for women with migraines to take hormone therapy
    • If hormone therapy will decrease the frequency of migraine headaches

    S2 Ep85: Non-Hormonal Prescription Drugs to Treat Hot Flashes Jul 13, 2023
Show notes The 2023 Menopause Society (NAMS) position statement regarding the non-hormonal treatment of Vasomotor Symptoms- hot flashes and night sweats- was just released. Position statements are expert analyses of current studies in order to determine recommendations based on science, as opposed to what the latest Tik Tok star or celebrity is promoting. Because new studies are constantly being published, there is always new information to inform those recommendations, which is why position statements need to periodically get updated.

Because the position statement is very extensive and long, I have divided it into 3 episodes.
    Part 1- Non-hormonal prescription options
Part 2 -Over the counter botanicals and dietary supplements
Part 3- Lifestyle; mind-body techniques; acupuncture and cooling devices.
In this episode, I am covering non-hormonal prescription options. My guest is one of the authors of The Menopause Society position statement, Dr. Chrisandra Shufelt. Dr. Shufelt is a certified menopause practitioner of The Menopause Society, the Chair of the Division of General Internal Medicine, Senior Consultant at the Mayo Clinic Women's Health, and Associate Director of Women's Health Research Center at Mayo Clinic in Jacksonville, Florida.
    The drugs discussed include:
    • Selective Serotonin Reuptake Inhibitors (SSRI) such as Paroxetine, Citalopram, Escitalopram
    • Selective Norepinephrine Reuptake Inhibitors (SNRI) such as Desvenlafaxine, Venlafaxine
    • Gabapentin
    • Pre-gabalin
    • Clonidine
    • Oxybutynin
    • Suvorexant
    • Fezolinetant
And stick around for the wrap-up at the end. I summarize the recommendations.

    S2 Ep84: Estrogen and Alzheimer’s Disease Jul 06, 2023
Show notes An estimated 6.7 million Americans are living with Alzheimer’s dementia and woman are twice as likely to develop Alzheimer’s Disease as men. So does hormone therapy increase the risk of Alzheimers, decrease the risk or have no effect? That question has been in the news this last week because of a new study that suggests that hormone therapy increases the risk..a finding that is exactly opposite to other studies. But is this new study valid?
In this episode, I break down:

    • The difference between perimenopause brain fog and dementia
    • What happens in the brain with Alzheimer’s Disease
    • Risk factors
    • Why estrogen is important
    • What the data shows in terms of the impact of taking hormone therapy

    S2 Ep80: WHEN PROGESTERONE is a PROBLEM Jun 08, 2023
    Show notes Your progesterone questions…Answered!
If estrogen is the good witch, for a lot of women, progesterone is the bad witch that causes bloating, weight gain, and moodiness. Some types of progestogens may also increase the risk of breast cancer. In this episode, I am going to go through the good, the bad, and all the options when it comes to protecting your uterus when using systemic estrogen therapy.
And since I get so many questions about progesterone, I included them all.
    In this episode, I cover
o Why a progestogen is prescribed when a woman takes estrogen post menopause
o The difference between a Progesterone, a Progestin, and a Progestogen
    o FDA Approved stand-alone progestogens
    § Medroxyprogesterone Acetate (Provera™)
    § Micronized Progesterne (Prometrium™)

o FDA Approved ORAL Progestogens combined with estrogen
o FDA Approved TRANSDERMAL Progestogens combined with estrogen
o BAZODOXIFENE-The only FDA approved non-progestogen to protect the uterine lining (and is back on the market!)
o OFF-LABEL options used to substitute for progestogens.
    § Levonorgestrol IUD
§ The problem with taking unopposed estrogen
§ The problem with compounded transdermal progesterone
§ The problem with transvaginal progesterone

S2 Ep79: Gender Magic with Rae McDaniel Jun 01, 2023
Show notes As a sexual medicine expert, I interact with people every day who are confused, mystified, and often put off by the very idea that someone does not define themselves by the sex they were assigned at birth. My guest isRae McDaniel - a non-binary therapist, certified sex therapist, transgender diversity and inclusion educator, and the author of Gender Magic- Live Shamelessly, Reclaim Your Joy and step into Your Most Authentic Self.
Gender Magic is not only a road map for individuals that do not neatly fit into a binary world but also for those who want to support a child, colleague, or loved one, or want to understand the transgender non-binary world better.
We start by defining the following terminology:
    • Cis Gender
    • Non-binary
    • Binary
    • Transgender
    • Transsexual
    • Queer
We then go on to discuss:
    • How a parent ideally would respond if either a very young child or an adult child reveals that they are non-binary or trans
    • Appropriate pronoun use
    • When the school doesn’t tell parents that their child identifies as non-binary or trans
    • The best way to come out to parents and other family members and friends
    • Frequency of regret after transition
    • Why a name change is important
    • The importance of solo sex and fantasy
    • The importance of therapy when transitioning
    • The concept of ethical curiosity
    • Why doing burlesque can be empowering!
    • What gender freedom is
