"I have been retained by Defendants and Plaintiffs' counsel as an expert in the area of dental standard of care. I have been a continually practicing full-time dentist for over 26 years and currently practice in Phoenix, Arizona. I have been a board approved consultant for over 20 years, adjudicating more than 1,000 dental board cases in Arizona. The primary responsibility of a board-approved consultant is to understand and define the standard of care and deviations from the standard of care. I have adjudicated cases that include failure to diagnosis, failure to treat dental conditions, dental fraud, inadequate treatment, over treatment, death of a patient, over sedation, inappropriate behaviors of a doctor and/or staff, inadequate clinical practice, inadequate treatment planning, appropriateness of care, and board generated cases due to malpractice settlements or awards. I have adjudicated cases in all fields of dentistry.

I was appointed by the Governor of the State of Arizona as their Dental Expert Witness for the Parsons's v. Ryan class action lawsuit in 2013. I also was chosen by the Attorney General of the State of Michigan as their Dental Expert Witness for their class action lawsuit (Johannes v. Heyns) as well as Dental Defense Expert in the Federal Detainee Case in Illinois (Smego v. Mitchell).

In addition, I currently consult and provide expert reports to the Arizona State Board of Dental Examiners. I developed the only board-approved template used by dental consultants and board members as a guideline to assess appropriateness of care.

Also, I have been retained as a dental expert witness in over 100 cases for both defendants and plaintiffs since the beginning of 2010 due to my expertise in the standard of care. I have over 3,000 hours of continuing education, including advanced training in prosthetics (both removable and fixed), dental implants, risk management, Occupational Safety and Health Administration (OSHA), Computer-Aided Design (CAD/CAM), Cone Beam Computer Tomography (CBCT), full mouth reconstruction, digital x-rays, endodontics, and lasers (both hard and soft tissue). I have been a beta tester for Dentrix, a dental software program commonly used by dentists throughout the United States, and I am familiar with other commonly used dental software programs.

Furthermore, I have lectured and done live patient demonstrations using both hard and soft tissue lasers. I have taught hundreds of dentists how to operate dental lasers, and I have published in restorative implant dentistry.

"DOCS" has utilized several suggestions I have made in establishing Standard of Care guidelines for sedation protocols as well as incorporated labels for emergency kits I help develop.

Additionally, I am a board-approved anesthesia evaluator and onsite inspection evaluator for the State of Arizona. I administer oral examinations and onsite inspections for anesthesia 1303 permits.

Lastly, I currently lecture on "The Standard of Care — What is it? What is it not?" as well as do Webinars for continuing education."

Dr. Sean K. Carlson is a Board Certified Orthodontist who received his dental degree from Harvard University in 1994. There he was awarded the American Association of Orthodontists Award upon graduation for demonstrating exceptional interest in the development of the oro-facial complex. He received both his Orthodontic Specialty training and his Master of Science degree in Oral Biology from the University of California at San Francisco in 1998.

Dr. Carlson is an Associate Professor of Orthodontics at the University of the Pacific School of Dentistry where he teaches one day a week. He has received several academic research awards including two American Association of Orthodontists Foundation Awards and has published numerous articles and abstracts in the orthodontic literature. He lectures nationally on a variety of clinical and theoretical subjects. His current lecture schedule focuses on three-dimensional imaging in orthodontics and the treatment of dental impactions.

Since 1998, Dr. Carlson has served as a Senior Investigator in the Craniofacial Research and Instrumentation Laboratory at the University of the Pacific School of Dentistry. His primary focus is on using computer technology to improve the way we study, teach and practice orthodontics. As a result of his research, he has been at the forefront of 3-D imaging technology since it was first introduced to the orthodontic profession. He installed Marin County's first i-CAT 3-D CBCT x-ray machine in 2008.

Dr. Jonathan Greenburg. My career as a Dentist has been truly fascinating and I believe is what has led me to where I am today. So where am I?

I am the founder and owner of Snore Experts which are 5 Treatment Centers in Los Angeles that exclusively treat Snoring & Sleep Apnea. They are where you would expect; Beverly Hills, Pasadena, Encino, Valencia and Newport Beach.

I am also the founder and CEO of Zyppah, Inc. which is "HAPPY Z" spelled backwards. Zyppah is a Company that has 2 parts: A Consumer Division and a Professional, both to Dentists and Physicians Division.

With my wife, I also wrote a book on Adventure Travel in the 90's called "If You Can't Remember Your Last Vacation, You Need This Book" which sold out its first 2 printings and for which we were interviewed on the Oprah Winfrey show.

The key component to Zyppah are the mouthpieces I invented to treat Snoring & Sleep Apnea.

My undergraduate degree was in Bio-Medical Engineering. I was actually one of the 1st Bio-engineering classes to ever graduate Syracuse from Syracuse University.

That was key to understanding how large a role the tongue plays in Snoring and Sleep Apnea and the fact that no one had developed a solution that was comfortable. Everyone just focused on bringing the jaw forward.

Dr. Thomas Shipley received his Bachelor of Science degree in Business Management from Brigham Young University and went on to earn his doctorate from the University of Kentucky's College of Dentistry. Dr. Shipley completed his master's degree in orthodontics at West Virginia University and has been providing outstanding orthodontic care to the Peoria, AZ community since 2005.

In order to stay up to date on the latest advancements in his field, Dr. Shipley is active in the Comprehensive Care Continuum Study Club, and he is the coordinator of the International Dental Ed Continuing Education Study Group for the entire Northwest Valley. Dr. Michael (Mike) Ragan is a native of the Dallas area and graduated from St. Thomas Aquinas Grade School and Bishop Lynch High School. He met his high school sweetheart and wife, Cindy at BL. He received his bachelor's degree from Baylor University where he was a baseball letterman for the Bears.

Doctor Ragan attended Baylor College of Dentistry, graduating with honors. He received his post-graduate training in orthodontics from Nova Southeastern University in Ft. Lauderdale, Florida, serving as chief resident. Doctor Ragan is a guest lecturer as well as the past Alumni president at Nova.

Doctor Ragan is a member of Baylor's Odontological Honor Society, Omicron Kappa Upsilon National Honor Society, the American Association of Orthodontists, the Southwestern Society of Orthodontists, Texas Association of Orthodontists, Greater Dallas Orthodontists Association, American Dental Association, Texas Dental Association, and Dallas County Dental Society.

Doctor Ragan and his wife Cindy were married in 1994. Cindy is a teacher of a four-year-old pre-kindergarten class at the Episcopal School of Dallas. They have two beautiful daughters, Abby and Caroline. Ragan enjoys sports of all kinds including hunting, fishing, softball, soccer, and running. He currently serves as the Athletic Department Dentist at the Episcopal School of Dallas and is a proponent of mouthguards for all ages, sports, and levels of competition.

Dr Armond finished dental school in Iran in 2001, during his 6 years of practice in Iran, opened 3 offices, sold them with profit and immigrated to the US in 2008.

Being in the states for only 4 months, passed the Board exam and started Work-ing in the Glidewell lab , the biggest dental lab in the united states, in the re-search and development section for a year where he was involved with the de-velopment of the Surgical guides for Implant placement and with various digital technologies including digital scanning and Zirconia crowns. Finished USC dental school in 2011, was a part time Faculty at the restorative de-partment and now owns and operates two offices in the Los Angeles area. Besides dentistry which is his great passion and also having worked as a dental lab technician, he has had acting roles in some movies in Iran, has performed the only rock concert in Iran as the lead singer and guitar player and speaks 5 lan-guages.
