    309 Residential Insights Improve Care – McCartney-2 Apr 18, 2019
    Show notes

Residential Insights - The Quiet Revolution

We care for kids from 6 to 17 in a residential environment with a variety of problems from misbehaviors at home or school - to suicidal ideation. At Barry Robinson we set plans from the outset to help each child achieve an improved sense of personal responsibility and self awareness in a balanced, caring environment. Our teams objective: to redesign the standards for residential care.
    ~ Robert McCartney

Rob's Residential Insights - His Quality Care Mission

As Executive Director of The Barry Robinson Center, a short-term 72-bed non-profit behavioral health system/residential program for both children and adolescents in Norfolk, VA, Rob sets a new, exemplary standard for residential care through improved residential insights.

In this interesting episode, Rob discusses important details in the evolution of family-oriented residential care, including his innovative program to dramatically decrease physical holding and how he abolished seclusion as a means of child/adolescent management.

You will appreciate hearing Rob's report on the evolution of his programs and how he manages to keep families together even though far apart. From the time they come in, he's planning on their going home.

Ed Note: This is a republish of an earlier interview with insights about the evolution of residential care.

    Rob's Most Significant Contribution

For years The Barry Robinson Center, here in Tidewater Virginia, only a few miles from our offices here at CoreBrain Journal and CorePsych, remained stuck in outdated thinking so prevalent even today in residential care facilities nationally.

That world has changed. Their programs work with clients both nationally and internationally.

Succinctly, Rob's treatment mission connects both individual/personal values with family evolution in innovated connected ways that help with the transition back home.

Rob's leadership coupled with his excellent team, as you will hear in this CBJ/309 Episode, brings an explicit, deeper, more thoughtful treatment mission to an already remarkable campus. Here he tells you why and how.

    The Toxin Solution Testimonial

From Dr. Mark Hyman, Chairman of Institue of Functional Medicine, Director of the Cleveland Clinic Center for Function Medicine:

Dr. Pizzorno's Toxin Solution is the definitive book linking the exploding burden of environmental toxins to chronic diseases including autoimmunity, obesity, cancer and more. The connection between toxins and disease is now undisputed - while ignored by the traditional medical community. For the first time, we have a clear roadmap of how these toxins impact our health, but more importantly a practical plan for reducing our exposures and maximizing our body's detoxification. This book should be a mandatory part of all medical school curriculums. And for any human living in the 21st century seeking to improve their health, this book is essential reading.

306 What Your ADHD Child Wishes You Knew – Saline-2 Mar 28, 2019
    Show notes

    Parent Alert! ADHD Parenting Evolves

Dr. Sharon Saline, Northampton, MA, Sharon Saline, a clinical psychologist and author What Your ADHD Child Wishes You Knew: Working Together to Empower Kids for Success in School and Life, specializes in working with kids, young adults and families living with ADHD, learning disabilities and other mental health issues. The upshot of her work is the instructive guide for ADHD Parenting.

Her unique perspective - as a sibling in an ADHD home, combined with decades of experience as a clinical psychologist and educator/clinician consultant - assists her in guiding families and adults towards effective communication and closer connections. She lectures and facilitates workshops internationally on topics such as understanding ADHD, executive functioning, different kinds of learners and the teen brain.

How: Dr. Saline's Helpful Advice To Reconsider ADHD Parenting - The 5 C's

Based on dozens of interviews from kids and families as well as clinical examples, this book shares the voices of kids with ADHD themselves and provides a parenting model that emphasizes practical approaches to everyday challenges and helps parents of all kids, not just those with ADHD.

My approach for ADHD Parenting progress is called The 5C's of ADHD Parenting - it relies on:

    • Self-Control,
    • Compassion,
    • Collaboration,
    • Consistency, and
    • Celebration

    Why

These daily practices reduce the frustration that most families living with the ADHD parenting experience. Kids with ADHD want to be included in discussions of plans aimed at helping them.

When they don't feel heard or considered, they lack buy-in and cooperation. They need direct instruction to improve executive functioning challenges and usually can identify areas where they need help.

Kids with ADHD want to be noticed for their efforts, not just their failures. They hear so much criticism that they don't believe feedback can be positive. It's often hard for them to get motivated or like themselves in light of the amount of negativity they receive.

Multiple Brain Integration Technique - Use Every Brain Effectively

Bill Lee Emery - Is a coach who joins us from The Gold Coast in Queensland, Australia for this interesting review of our three brains, - what they do, and how to use them effectively. He is an mBIT - Multiple Brain Integration Technique - Coach and mBIT Master Trainer - and his favorite tribe-folk are heart-based entrepreneurs. [Is there an entrepreneur out there who isn't heart based?]

This interview will take you to places you intuitively already knew, and then adds clear maps with improved brain function markers to improve your response accuracy and timing.

Ed. Note: This is a republish of CBJ/207 because it's a remarkably useful tool for any providers understanding ways to address previously unrecognized perceptions.

Photo by Vlad Tchompalov on Unsplash

Listen To Bill Explain How Each Brain Works

mBIT, Mulitple Brain Integration Technique, offers unique insights and highly efficient processes you can easily train to use to overcome these kinds of challenges in your life.

Through a series of relatively simple yet powerful techniques, you can balance your autonomic nervous system, allowing you easier access to communicate with your three brains.

Once you are connected and communicating with your three brains, you now have the opportunity to align them, i.e., get them in agreement with each other. No more running in different directions. And here’s where things start to become fascinating.

    Congruency Matters

Communication and alignment of your brains are vital for congruence, success, and happiness in life. Imagine how incredibly life-enhancing it is when your many minds are working harmoniously together to produce generative wisdom in your life!

Having your brains aligned allows you to quickly and effortlessly tap into the innate wisdom that resides inside you. You will be able to resolve internal conflicts so that you feel more congruent in your life, bring deep intuitive sense into your decision making and action taking, connect to yourself and others in ways you may not have experienced before and create wise and compelling goals, dreams and outcomes.

A Brief Multiple Brain Integration Technique Overview - The importance of each brain’s Prime Function

Each mind has a defined, distinct job to do. Just like in a company or organization where different people have different roles to perform, each brain has specialized tasks.

And yet, if we don’t know what these prime functions are or how to access them efficiently and optimally, it’s highly likely that we could find ourselves experiencing all kinds of internal conflicts and struggles in life.

Perhaps you can relate to situations like these:

    • You’re not even hungry, and you’re eating anyway. Your head knows it’s a futile attempt to fill the emptiness inside and yet you can’t stop yourself from devouring the last of the chocolate cake!
    • You’re making goals and plans for your life that feel exciting. And then, you sabotage your dreams. Fear blocks you from moving forward, or procrastination holds your intentions at bay. It’s frustrating, and you just don’t understand why you can’t move forward consistently.
    • You feel confused about an important issue in your life because your head says one thing, your heart says another and your gut is merely churning over it all and making you feel sick. There’s internal conflict between your thoughts, feelings, and action.

These kinds of situations are common when the three brains are not aligned and not working well together, or where one brain might be trying to fulfill the role of another. It’s also possible for one or more of your three brains to be in conflict or antagonism with another. And it’s times like this when we find people experiencing struggle in their life because their brains are fighting each other or merely ignoring each other.

    Each Brain's Prime Functions:

    • Head Brain
    • Cognitive Perception
    • Thinking
    • Making Meaning
    • Heart Brain
    • Emoting/Deep Feelings
    • How we connect with people
    • Values
    • Gut Brain
    • Identity
    • Self-preservation/safety
    • Mobilization/capacity to act

The Importance of Each Brain’s Prime Function

Each brain has a clear, distinct job to do. Just as in a company or organization where different people have different roles to perform, each brain has its specialized tasks.

And yet, if we don’t know what these prime functions are or how to access them efficiently and optimally, it’s highly likely that we could find ourselves experiencing all kinds of internal conflicts and struggles in life.

Once you understand and can assess/recognize the patterns, then Multiple Brain Integration Technique can create more efficient, actionable, balanced outcomes.

    304 Educational Testing Dyslexia & Social Justice – Stein -2 Mar 14, 2019
    Show notes

Educational Testing, Social Justice, Money & Labels

We cannot solve our problems with the same thinking we used when we created them.
    ~ Albert Einstein

Dr. Zak Stein - Dr. Stein's educational testing interests focus on the interface between social justice and education, including specializations in developmental psychology and ethics. He studied philosophy and religion at Hampshire College, and then educational neuroscience, human development, and his doctorate in philosophy of education at Harvard University.

While a student at Harvard, he co-founded what would become Lectica, Inc., a non-profit dedicated to the research-based, justice-oriented reform of large-scale standardized educational testing in K-12, higher education, and business. He writes on a broad range of topics including the philosophy of learning, educational technology, and the fundamental theory of education translated to applications for everyday life.

Neuroscientifically validated details also matter for educational testing. Current testing requirements, including IQ, don't adequately cover mind complexity. Listen and comment. Consider: should the term for current standards of educational testing results and their subsequent applications with students more accurately be characterized by the term social injustice?

    Educational Testing Evolves

Testing doesn't adequately cover mind complexity. Imprecise results create myriads of challenges for everyone concerned - from students to teachers to school administrators and parents.

Currently, Dr. Stein serves as Chair of the Education Program at Meridian University, and as Academic Director of the activist think-tank at the Center for Integral Wisdom.

Ed Note: Daniel Schmachtenberger [http://corebrainjournal.com/084], our respected mutual colleague at Neurohacker, is friends with and referred Zak.

Bottom line: our system of education-labels in the context of new measurements, new data and evolved mind considerations is woefully inadequate and potentially damaging to the lives of those mislabeled. This discussion is critical, indeed absolutely essential, dare I say mandatory, for anyone involved with any educational process. Feel free to weigh in below with a comment.

Connect With Dr. Stein To Encourage Systemic Evolution

"I am available for speaking engagements on a variety of topics, including Mind, Brain, and Education, Developmental Psychology, Human Development, Philosophy of Education, Social Justice, Educational Psychopharmacology, Educational Testing, and Integral Meta-Theory. I’ve been an invited speaker and workshop leader at venues such as Harvard University, The National Security Agency, Integral Institute, Buckingham Browne & Nichols, Amherst College, and a variety of academic conferences.

At times I also offer consulting services to schools and other educational institutions around assessment, philosophical alignment of mission, vision, and practice, and developmental psychology. It has been an honor to have served in various consultancy roles with organizations such as the Department of Mind, Brain, and Education at the Harvard University Graduate of Education, Brandies University, Phillips Exeter Academy, the Long Trail School (VT), and Glastonbury Public Schools (CT).

I occasionally provide academic coaching to graduate students and professional writers, helping them frame problems, build a course of study, conceive a research project, or simply manage their energy and stress during the completion of complex academic tasks.

I'm not a life coach or ghostwriter; but if you come to me with a passion, I will help."

    303 Essential Tremor – High Intensity Focused Ultrasound – Kaplitt-2 Mar 07, 2019
    Show notes

Essential Tremor Treated With High-Intensity Focused Ultrasound

Dr. Michael Kaplitt is the first doctor in New York to use high-intensity focused ultrasound [HIFU] to relieve a patient’s Essential Tremor - ET. The procedure is part of a new clinical trial testing the use of this technology to eliminate the source of tremors in a completely non-invasive way. Dr. Kaplitt and his neurosurgical team, along with Dr. Levi Chazen from Radiology and Dr. Harini Sarva from Neurology, were able to watch as the patient’s tremor diminished visibly during the procedure. [Listen to his patient Steve here at CBJ as he tells you what it felt like coming out of HIFU treatment.]

Essential tremor may be mild in some individuals, but in others, it can become debilitating over time. The tremors, which are often most severe in the hands, can interfere with everyday activities such as eating, drinking, writing or tying shoelaces.

Ed Note: This is an important republication fromCBJ/232.

Patient Experience with Dr. Kaplitt's Team

We asked for this interview after working with For a previous CBJ report from a healed golfer-patient treated by Dr. Kaplitt with HIFU listen here: CBJ/221. This Ultrasound tool and the Tremor Treatment Team at Weill Cornell Brain & Spine is simply amazing.

Current treatment options for ET - Essential Tremor include medication as well as Deep-Brain Stimulation (DBS), which involves implanting a battery-operated neurotransmitter under the collarbone connected to a wire that runs up the length of the neck into the scalp, where it is guided to the brain through a small hole in the skull. The tip of this wire sends the electrical impulses generated by the neurotransmitter into the precise spot in the brain that regulates the activity of the key circuits in essential tremor. In a phrase, brain surgery.

Michael Kaplitt MD, Ph.D., and the Weill Cornell team are also national leaders in the use of DBS for essential tremor, Parkinson’s disease, epilepsy and a variety of other disorders for more than 15 years.

Listen up as Dr. Kaplitt discusses with us this remarkable new, non-invasive procedure to correct ET without brain surgery. - And he answers some of my questions about Parkinson's as well.

Photo by Nathan Dumlao on Unsplash

    Additional Research Work

Dr. Kaplitt has pioneered human gene therapy for neurodegenerative disorders, having performed as a student the first rodent study that used the adeno-associated virus (AAV) vehicle for gene transfer to the brain.

He performed the world’s first human gene therapy procedure for Parkinson’s disease and more recently helped to guide the first successful randomized, double-blind, sham-controlled trial of this experimental treatment, which was the first positive trial of this kind for gene therapy in any brain disorder.

His Psychiatric Side Beyond Essential Tremor

He is also pioneering the development of novel deep brain stimulation treatments for psychiatric conditions, including refractory drug addiction. Dr. Kaplitt is an internationally recognized expert in the science of gene therapy and molecular neurobiology, and his current focus is to understand molecular disease mechanisms and develop novel molecular therapies for Parkinson’s disease, depression, addiction, and pain.

He has published over 100 articles and has edited two books on these subjects, with publications from his lab appearing in the most prestigious international journals, including Science, Science Translational Medicine, Nature Genetics, Nature Medicine, Proceedings of the National Academy of Science and Lancet. His basic science lab has received funding from the National Institutes of Health since 2004 and has also been the recipient of major funding from the Department of Defense and several major private foundations.

Dr. Kaplitt is the recipient of numerous awards, including the Young Investigator Award from the American Society for Gene Therapy. In 2004 he was named to Crain’s 40 under 40 list of 40 young New Yorkers and in 2015 he was elected to membership in the prestigious American Society for Clinical Investigation, the honor society for scientists focused upon clinical and translational research. He has also been elected to membership in the Society for Neurological Surgeons, the honor society for senior neurosurgeons.

And on top of all of these credentials, he's a great guy to interview on these ground-breaking treatment tools. You will enjoy this conversation.

    302 On Biomedical Evidence – Hedaya-2 Feb 28, 2019
    Show notes

Biomedical Evidence - Hedaya on The Biology of Mind and Body

The important thing is not to stop questioning. Curiosity has its own reason for existing.
    — Albert Einstein
Everyday we increasingly confirm that inflammation directly effects brain function. Over time it can create permanent change.
    - Robert Hedaya

Treatment Failure Without Biomedical Evidence Treatment failure, especially in the face of proper training and the best information, means you're/we're missing something. We're missing mind complexity. It's not the patient's fault. It's not the doctor's fault. Looking to blame someone? Blame our historical dependency on behavioral appearances.

The inadequacy of reductionistic labels denies the complex reality of mind, body and biomedical functional complexity. The system is set to run on labels and medications that target those dancing clouds of surface appearances. We're living in akimbo-land, with a frustrating global problem of misperception in the context of current scientific details.

It's not that the current psychiatric system is bad or broken - it's excellent - and yet the labels system remains merely inadequate in the context of fresh data from modern neuroscience. Repeated imprecision builds disdain, distrust, and stigma.

...misunderstandings and neglect create more confusion in this world than trickery and malice. At any rate, the last two are certainly much less frequent.
    ~ Goethe - 1774
Don't assume bad intentions over neglect and misunderstanding.
    ~ Hanlon's Razor - 1990

Dr. Hedaya unearthed this flash of insight many years ago, back in 1985, and it continues to drive him, as he's still searching today. In this episode, Dr. Hedaya shares his personal story and tells us how he turned his medical, psychiatric curiosity in a different direction. Hedaya emphatically encourages more critical thinking - with more systematic reviews of dynamic change, time, and context. More careful biomedical reviews do inevitably change the rules of the game.

Listen while we discuss just how he became one of the leading national authorities in that interesting space, the polarity, between traditional psychiatry and functional medicine. Also, listen carefully to how often we both bring up inflammation as an important marker on the crossroads between these two often adversarial diagnostic and treatment strategies.

Ed Note: Dr. Hedaya continues in active practice, and, also, remains firmly involved in mind research. In this interview, he discusses the relevance of pain and inflammation in his own life, and the discoveries he's made on his clinical journey into the challenging territories of dementia, including TBI and Alzheimer's. Notice how we segue into IgG measurements, and the challenges of other mind inflammations found downstream from food sensitivities.

Note also that this is a re-publication from CoreBrain Journal/005 very soon after we started this Journal three years ago, and it is not outdated. Dr. Hedaya is an internationally prominent functional medicine psychiatrist who teaches at Georgetown, well worth a listen. Inflammation matters.

His Message In A Sentence: Every treatment failure, every refractory, chronic mind imbalance deserves specific biomedical measurements for contributory factors, including inflammatory processes. Thanks, Bob!

Adams, Ph.D., is a President’s Professor at Arizona State University, where he directs the autism/Asperger’s research program, though he originally taught chemical and materials engineering there. Dr. Adams also holds a post at the Southwest College of Naturopathic Medicine. He is also the president of the Autism Society of Greater Phoenix, the co-chair of the Autism Research Institute’s scientific advisory committee, and has received the Autism Service Award from the Greater Phoenix chapter of the Autism Society of America. In this episode, he reports on microbiota transfer therapy.

This CBJ/301 is a republication of CBJ/214 and is selected because it reveals a most interesting development in Autism Treatment directly related to gut microbiota and his research with FMTT Fecal Microbiota Transfer Therapy. Must listen!

Microbiota Transfer Therapy alters the gut ecosystem and improves gastrointestinal and autism symptoms: an open-label study.

    Background

Autism spectrum disorders (ASD) are complex neurobiological disorders that impair social interactions and communication and lead to restricted, repetitive, and stereotyped patterns of behavior, interests, and activities. The causes of these disorders remain poorly understood, but gut microbiota, the 1013 bacteria in the human intestines, have been implicated because children with ASD often suffer gastrointestinal (GI) problems that correlate with ASD severity.

Several previous studies have reported abnormal gut bacteria in children with ASD. The gut microbiome-ASD connection has been tested in a mouse model of ASD, where the microbiome was mechanistically linked to abnormal metabolites and behavior.

Similarly, a study of children with ASD found that oral non-absorbable antibiotic treatment improved GI and ASD symptoms, albeit temporarily.

Here, a small open-label clinical trial evaluated the impact of Microbiota Transfer Therapy (MTT) on gut microbiota composition and GI and ASD symptoms of 18 ASD-diagnosed children.

    Results

MTT involved a 2-week antibiotic treatment, a bowel cleanse, and then an extended fecal microbiota transplant (FMT) using a high initial dose followed by daily and lower maintenance doses for 7–8 weeks.

The Gastrointestinal Symptom Rating Scale revealed an approximately 80% reduction of GI symptoms at the end of treatment, including significant improvements in symptoms of constipation, diarrhea, indigestion, and abdominal pain.

    Conclusions

This exploratory, extended-duration treatment protocol thus appears to be a promising approach to alter the gut microbiome and virome and improve GI and behavioral symptoms of ASD. Improvements in GI symptoms, ASD symptoms, and the microbiome all persisted for at least 8 weeks after treatment ended, suggesting a long-term impact.

Similarly, clinical assessments showed that behavioral ASD symptoms improved significantly and remained improved 8 weeks after treatment ended. Bacterial and phagedeep sequencing analyses revealed successful partial engraftment of donor microbiota and beneficial changes in the gut environment. Specifically, overall bacterial diversity and the abundance of Bifidobacterium, Prevotella, and Desulfovibrio among other taxa increased following MTT, and these changes persisted after treatment stopped (followed for 8 weeks).
