211 Myofunctional Therapy Series part 3 with Sarah Hornsby Jan 22, 2020
    Show notes

A Tale of Two Hygienists continues its series on myofunctional therapy by looking at how these issues manifest in older children and adults and how early we should act in order to most effectively treat them. Joining Michelle again for this series is Sarah Hornsby!

Sarah K. Hornsby, RDH, is a myofunctional therapist, speaker and mentor. Her interest in myofunctional disorders arose from her own myofunctional problems, inspiring her to ask bigger questions about why these issues are prevalent in today's society. This ultimately led to her launching her own myofunctional therapy practice, Faceology, a decade ago.

In this episode, Sarah discusses a whole new movement in orthodontic treatment, examines how myriad factors do—or do NOT—impact myofunctional health, weighs the benefits of techniques for treating myofunctional issues while you sleep.

    EPISODE HIGHLIGHTS:

    Interview starts: 15:59

    - Sarah discusses a controversy in the orthodontic community over when certain teeth should be removed.

    - How Invisalign is branching out its products to a younger age group.

    - The questions Sarah asks the orthodontists she works with.

    - Looking at the relationship between mouth-breathing and rampant cavities.

    - Taking a comprehensive approach to diagnosis myofunctional issues rather than jumping to conclusions based on one or two traits or predictors.

    - A quick and easy method to screen patients for potential myofunctional issues.

    - Why myofunctional therapy involves higher carbon dioxide intake.

    - CPAP: a useful treatment, but definitely not the cure.

    QUOTES:

"I really appreciate those dentists who are trying to start early."

"If you look at all the people with the worst perio, they probably have myofunctional issues."

"If you change the teeth but not the muscles and the soft tissue and the habits, of course the teeth are gonna go back."

"Myofunctional therapy is really just physical therapy for the muscles of the mouth."

"Especially with our TMJ patients, we have to be including some sort of myofunctional component or they're not gonna get better."

    EPISODE HIGHLIGHTS:

    Interview starts: 14:15

    - Sarah explains how nutrition is not the only important part of breastfeeding.

    - Why it's better to perform a tongue tie procedure as early as possible.

"I think any baby who has acid reflux... should definitely be screened for a tongue tie."

"Our bodies are amazingly resilient and adaptable."

"Education is the key. If moms know, they can make a decision, and that's it."

"Mouth-breathing alone, at night, will affect your sleep."

"Do your own research, and if you feel like something's wrong, trust your gut."

208 Student Focused Presents: Treating Patients with HIV Jan 01, 2020
    Show notes

For the latest student roundtable, A Tale of Two Hygienists has invited on three guests who either work with patients affected by HIV/AIDS or have studied the virus to debunk myths and misconceptions surrounding this epidemic.

Daniel Lopez has been a hygienist since 2012; since graduating, he's been working with Access Community Health Center in Manhattan, which focuses on LGBTQ patients and the treatment of HIV. Amber Riley is based in San Diego and has been a dental hygienist for over two decades and works in a suburban practice; a small portion of her patients live with HIV and she's seen firsthand how the prognosis of those living with the virus has changed over the years. JoAnn Gurenlian has over 40 years of dental hygiene experience under her belt; she is the graduate program director at Idaho State University, and back in the 1980s she was part of a task force that investigated HIV/AIDS during the initial outbreak.

In this roundtable, our guests discuss how we can prevent exposure to and the spread of HIV, explain the importance of "patient navigators" and how oral health care professionals should adjust their treatment for patients with HIV/AIDS.

    EPISODE HIGHLIGHTS:

    Interview starts: 3:05

    - JoAnn breaks down the difference between quantitative and qualitative studies and the benefits of each.

    - What treatments and methods can be used to prevent exposure to and proliferation of HIV?

    - JoAnn discusses the positive correlation between patients' sexual orientations and their health care providers' own orientations.

    - The ins and outs of HIV PrEP (Pre-Exposure Prophylaxis): prescribing, side-effects, and what advancements are being made.

    - Who are "patient navigators" and why are they so essential for helping people get the treatment they need?

    - How Maslow's hierarchy of needs can determine whether or not people seek out dental treatment and care.

    - Dispelling the idea that HIV contraction is the result of promiscuity.

    - What does—and doesn't—change for our guests' usual standard operating procedures when they learn a patient has HIV/AIDS.

    QUOTES:

"We're pretty much the future model of what health care is trying to shift towards."

"For anyone who feels that their risk of exposure can be high, this is a preventative procedure."

"We need to find a way to talk about risk in a positive way."

"Sexual health is absolutely demonstrated in the oral cavity."

"Every generation is less toxic than the last."

"If someone is virally suppressed, there are no documented cases of them transmitting the virus."

"You have to learn to be discerning about what is good research."

"Every person is worth all of our time and attention."

    Book: The Miracle of Health

207 Airway Health with Julia Worrall! Dec 26, 2019
    Show notes

Sometimes the real challenge in treating an airway disorder is getting the patient to actually take our advice—which can mean encouraging the other care providers a patient has to speak to them about the problem. This week's guest believes in not only treating airway disorders within our profession, but reaching out to other medical fields to ensure these problems are comprehensively addressed.

Julia Worrall, "The Sleep RN," is a registered nurse and the program director of the FAH Airway Advocate Program, which advocates for greater awareness and treatment of airway issues. A member of the National Commission on Correctional Health Care, she works tirelessly to address the epidemic of sleep disorders affecting Americans today.

In this episode, Julia explains how hygienists can sound the alarm on airway issues, discusses how dental professionals can link up with people in other fields to tackle these problems, and suggests ways we can more effectively reach out to patients about airway concerns.

    EPISODE HIGHLIGHTS:

    Interview starts: 16:30

    - Why Julia really appreciates the learning curve aspect of her work, even if it means having to discard what she thought to be true.

    - Overcoming our natural impostor syndrome as the dental community takes on more and more medical issues.

    - How dental professionals can become "first responders" when it comes to airway issues.

    - Why how we communicate our airway concerns to patients can affect if they actually take our advice.

    - On building bridges with nurses and other members of different medical professions.

    - How our knowledge of airway issues has changed over the course of 2019—and what Julia says 2020 has in store for us.

    - Getting over your fear of admitting "I don't know."

    - What's the most interesting thing Julia has learned at this conference?

    QUOTES:

"We're also professionals and that means we don't stay stuck in what we believe to be true when there's new evidence."

"You have to balance your time with what you're teaching and what you're learning."

"Own your space, own your knowledge."

"Each patient that you have deserves 100%."

"I wanna help you guys get to where you're seen, you're heard, you're validated."
