    The Future Of Robotic Surgery Aug 09, 2023
    Show notes In this episode of The Future Of, Caleb Osborne, Director of Product Development at Asensus Surgical, joins Jeff Dance to discuss the future of robotic surgery. They unveil the types of robots involved in surgeries today, the newest developments in surgical robotics, and the surgeries robots are already doing.

    They Cover:

    • [05:20] - As a smaller and more focused group, Asensus has seized the opportunity to concentrate on key areas. Their instruments boast high reusability, resulting in low per-procedure costs and setting industry standards. In contrast to typical 7.5 to 8-millimeter robotic instruments, LUNA's five-millimeter instruments are best-in-class, crucial for converting traditional laparoscopic procedures to minimally invasive ones. Notably, Asensus leads the way with the Intelligent Surgical Unit (ISU), offering real-time interoperative video and data access, replacing outdated methods like sterile measuring tapes, and opening up a world of clinical possibilities.
    • [10:10] - Surgical robots come in various types, each specialized for specific medical purposes. Future robots may serve only one or two segments of surgery. Examples include endovascular steerable catheter and bronchoscopy robots, as well as neurosurgery, spine, and orthopedic robots. Some procedures are image-guided, while others use telemanipulation. Fully autonomous soft tissue surgery is still challenging due to anatomical variations, making spine and orthopedic surgeries more amenable to autonomy. Nevertheless, robotics and AI enhance surgical outcomes in specialized fields.
    • [16:48] - Advancements in surgical robotics driven by AI offer great promise for medical practices. However, it's vital to address concerns around privacy and cybersecurity. Fortunately, the FDA has taken steps to regulate the use of AI in medical devices. Their requirements include rigorous testing with specific datasets to ensure consistent performance, as well as a clear statement of the intended use for each AI algorithm in specific procedures. The FDA also distinguishes between "closed" and "open" algorithms. Closed algorithms are fixed, while open ones can adapt to new procedures.
    • [24:00] - Caleb envisions the future of robotic surgery, where technology and expertise are seamlessly integrated. Surgeons will use robots in the operating room and confidently present the system to patients, creating trust in their skills and the robot's capabilities. To make this happen, extensive training and ongoing support will be essential. Additionally, robotic systems will integrate well with other medical devices, creating an interconnected operating room. This collaboration, especially between the robot and anesthesia technology, will improve patient outcomes. The focus is optimizing the entire surgical process while keeping the surgeon's experience efficient and effective.
    • [38:08] - The need for high-bandwidth data traffic in and out of operating rooms (OR) is driven not only by surgery but also by the opportunity to use other needs, such as diagnostic imaging and electronic health records. It is important to manage data flow and privacy across countries like Germany and Italy, especially in the cloud. Fortunately, major companies like AWS and Google Cloud are already working on privacy solutions and plugins. However, regulatory bodies must support these advancements to ensure seamless integration of cloud computing and on-device operations, which can enhance the clinical experience. Just like they have done with cybersecurity, they must now address privacy and AI issues collaboratively and promptly.
    • [51:15] - In the medical device field, we often overlook two critical aspects: the overwhelming number of devices clinicians handle daily, each with numerous buttons and screens. This complexity can inhibit their ability to care for patients effectively. For surgeons working with multiple robotic systems, while login procedures may vary, core interactions like tissue manipulation should be kept consistent. As technology advances, we should shift competition towards innovative features. This parallels how backup cameras became standard in cars. Additionally, we can provide hospitals with resources for patients, such as simplified simulators to build trust between patients and surgeons.

    • [11:37] - To ensure robotics safety regulations keep up with technological advancements without compromising human safety and ethics, companies should invest time upfront in hazard analysis. By understanding the scope of safety challenges, they can prioritize and solve high-priority issues while gradually addressing others. It's crucial to involve domain experts and tailor regulations to specific applications. Anticipating and acknowledging potential problems, even without immediate solutions, demonstrates a proactive approach. Considering security alongside safety is vital. By thinking ahead, organizations can strike a balance between innovation and responsible development, fostering robotics advancements while upholding safety and ethical standards.
    • [19:03] - The rapid rise of generative AI has profound implications for robotics. It goes beyond visual and text-based models, enabling robots to see, identify, and perform tasks more effectively. However, this progress necessitates a focus on safety without suppressing innovation. Trust and predictability are crucial, as humans rely on established rules for cooperation. Implementing oversight and guidelines ensures the responsible use of AI.
    • [22:42] - Nathan raises an important concern regarding oversight and disclosure in the autonomous vehicle industry. Companies often fail to be transparent about accidents, leading to skepticism. Nathan suggests that companies should openly share information, even when accidents occur, to build trust. The line between humans and robots blurs further with concepts like the Dr. Octopus suits, showcasing the significant overlap between these two realms.
    • [07:00] - In the past, companies faced challenges with the complexity and cost of using robotics, hindering their adoption. However, the industry has made significant strides in addressing this issue by developing user-friendly robotics solutions that are easy to implement. While system integrators are still necessary in certain cases, simplified setup processes are becoming more prevalent. This emphasis on ease of use is crucial as industries aim to automate their operations. Additionally, the impact of AI, including technologies like ChatGPT and Machine Learning, is a significant topic of discussion. Moreover, the perception of robotics and automation has shifted, with small and medium-sized companies realizing that these technologies are not exclusive to large corporations like General Motors.
    • [13:24] - The initial fear that robots would eliminate jobs in the US automotive industry did result in significant job losses when the robot industry faced a downturn. However, the real threat to jobs arises when companies can't compete, putting all jobs at risk. In the past two decades, we witnessed a substantial shift of US jobs to Asia due to the pursuit of cheap labor. Ironically, China, now the world's largest robotics user, relies on robots to maintain its manufacturing dominance. This strategic focus on robotics allows them to excel in quality, delivery speed, and productivity. Despite concerns about replacing jobs, history has shown that technologies enhance productivity, create new jobs, and offer better pay and safety.
    • [23:03] - Staying updated on robot safety standards is crucial in the robotics industry, with the Association for Advancing Automation and ISO providing valuable support in this regard. Actively engaging and keeping oneself well-informed about these standards is essential, as ensuring the safety of robots forms a fundamental foundation of our industry. It is of utmost importance to prevent any harm to humans caused by robots and address any concerns people may have. Thanks to decades of dedicated work, we have achieved excellent safety standards that have effectively prevented incidents involving humans and robots.
    • [29:15] - Service robots have garnered significant attention recently, as they hold immense potential to assist us with various tasks, including elder care. We are gradually advancing in this field. The logistics industry has already showcased the effectiveness of mobile robots on a large scale. The deployment of autonomous mobile robots in warehouses and factories has demonstrated their versatility in diverse settings. Hospitals now employ robots to transport supplies from storage areas to nurses, enabling them to stay close to their patients. Likewise, in restaurants, waiter robots efficiently deliver food to tables, allowing human staff to focus on other aspects of service and ensuring a seamless dining experience.

    • [05:57] Cloud is basically renting a computer and power from someone else regularly. Multi-cloud uses multiple public clouds. If you're using Amazon and Google, you're multi-cloud. Hybrid cloud or hybrid IT would also then incorporate your on-premises private cloud. Serverless is function-as-a-service, where you deploy this function somewhere else, it's only going to run once in a while, and it's cheap.
    • [10:45] Cloud 1.0 was the first version, which included a server, some networking, and storage. Cloud 2.0 was when the IaaS vendors started delivering differentiated services. Cloud 3.0 is more about turning that into a platform, and IaaS vendors provide different services for the developers to hook into.
    • [14:35] More than 90% of computers' workloads are done in the cloud today, and that's because you can't compete with the cloud. It's not on cost or agility but on scale, reach, and business continuity.
    • [21:44] Cloud computing has witnessed several notable advancements in recent times. Firstly, there is a heightened focus on the development of artificial intelligence (AI) and how cloud providers are contributing to its growth. Secondly, there is a renewed emphasis on infrastructure improvements to enhance the speed and efficiency of various cloud-based technologies.

    • [00:10:55] - When AI and machine learning started to become popular, it was more about looking at historical data and trying to predict something new about the new unseen data. Generative AI is a bit different as it could generate text, video, or audio content. Instead of just predicting, it can't create new data and content that could be hard to distinguish from other humans.
    • [00:17:51] - Open AI is one of the most well-known companies in the generative AI space. And other big tech companies like Microsoft, Adobe, and Google are implementing their own versions, or they've accepted the technology and started to build with it themselves. For every major technology company, you can see elements of generative AI. For example, DALL-E, Stable Diffusion, and Midjourney are shaking up the design and art space.
    • [00:26:04] - Stanford has said that "these new forms of generative AI have the capacity to change how we think, create, teach, and learn. They may also change our perspective on what is important to learn." There is an unlimited number of generative AI use cases across a wide variety of industries, helping them go from zero to one faster.
    • [00:39:21] - Generative AI still faces many limitations, such as the high cost of use and turning it into a service, the number of queries you can run, technical constraints, and unreliable or biased outputs. But the industry evolves fast, and most of these limitations could be overcome shortly.
    • [00:48:20] - Change itself is exponential, and most people have no idea what it means. Even for someone in this field, it's hard to understand where things are headed and what the future will look like. So looking ten years into the future, at this point, it's almost impossible to predict the magnitude of change. However, we can be sure that change will happen, generative AI will evolve, and we have to keep pace with this shift.
    • [00:56:41] - Generative AI is a technology that can create and disseminate more value among people, leading to an overall increase in the economy. While it may displace or transform some jobs, it will also create new opportunities. In the realm of software engineering, the gap between beginners and experts may widen due to the transformative power of generative AI. Additionally, smart assistants are poised to become integrated more deeply into our lives and will personalize their services to better assist us.

    The Future Of Aging Mar 30, 2023
    Show notes Dr. David Leonardi, Founder of the Leonardi Institute, joins Jeff Dance to discuss the future of aging, including approaches to prevent and reverse age-related diseases, the difference between aging and vitality, and lifestyle habits to slow the aging process.
    They Cover:

    • [00:06:56] The scientific aspect of aging involves changes in DNA and the epigenome that cause debility within cells as well as the top causes of death today, which are coronary heart disease and stroke followed by cancer. Coronary disease and cancer account for about sixty-seven percent of mortality.
    • [00:11:12] The classic approach to disease was to look at a single one and reverse it after it's already been established. A much better way focuses on altering chemical pathways before people get sick so that they can delay or potentially indefinitely prevent diseases from occurring while maintaining their vitality instead of getting debilitated before death. That is where the scientists are focused right now.
    • [00:12:32] When you talk to people about living beyond 100 years old, many immediately picture an ancient person with a three-foot-long beard sitting in a wheelchair in a nursing home. When you slow the aging process, you maintain your healthy lifespan. The scientist's plan for aging is to retain our youth instead of getting debilitated before death so people can enjoy a healthy and active life.
    • [00:21:27] An alternative version of recent breakthroughs in reversing aging is the information theory of aging proposed by David Sinclair. According to this theory, aging is not caused by defects in our genes but rather by changes in the epigenome, which is how our genes are wound around each other. One of Sinclair's students, Yingling Liu, experimented with the Yamanaka factors to reverse aging in mice. However, this resulted in the development of cancer. To address this issue, Yingling eliminated one of the Yamanaka genes responsible for cancer and used the other three to transform old cells into pluripotent stem cells, which did not result in cancer.
    • [00:34:09] To slow the aging process, Dr. Leonardi suggests a low glycemic diet with minimal sweets and starches, plant-based proteins, and fats. He also encourages people to avoid toxins such as saturated fats and trans fats, limit exposure to air pollution, regularly exercise, use nutritional supplements that affect pathways like AMPK (Adenosine Monophosphate Kinase), intermittent fasting to activate AMPK without caloric restriction, NAD chemical messages out from the nucleus to invigorate mitochondria, autophagy, and mitophagy which involve recycling old proteins into active, healthy compounds for use in cells. Dr. Leonardi emphasizes having clean drinking water by using reverse osmosis filters to filter out toxic minerals like iron, copper, mercury, and cadmium from drinking water.

    The Future Of Advanced Biometrics Mar 07, 2023
    Show notes Micah Willbrand, Chief Product Officer and VP of Enterprise at NEC Corporation of America joins Jeff Dance to discuss the future of advanced biometrics, including some common types of advanced biometrics currently in use, main ethical concerns, and how advanced biometrics will evolve in the next ten or twenty years.
    They Cover:

    • [00:03:07] Micah is a visionary leader that leads pragmatically. He states that many tech companies fail because they are too operationally focused or have a product the market won’t accept for the next five or six years. At NEC, he tries to build a hyper-focused consumer culture and create privacy and consent-focused products and services that will make people's lives easier.
    • [00:06:06] People are familiar with biometrics like fingerprinting and facial biometrics to access smartphones. When we talk about advanced biometrics, there are two factors to consider. Firstly, biometric tech in labs has tremendous accuracy. But the second aspect is how we get biometrics because you need something to capture it. So we're seeing new techniques, like photoacoustic imaging, which works through lasers and ultrasound. The future of advanced biometrics brings various applications, including contactless fingerprints, which will create a more device less world.
    • [00:11:06] NEC technology helped identify the Boston bomber. Regarding government and security, the company helps do the biometric matching for Border Patrol to ensure that the person entering the US is the same person on the passport. In the aviation industry, we see more of the so-called "ticketless journey." For example, Delta has multiple gates where you can board with just your face. Thanks to advanced biometrics, we'll see an outgrowth in protection against fraud.
    • [00:18:38] When consumers hear biometrics, there are multiple ethical concerns. It's one of the barriers for the market to accept this technology. One of the NEC's core pillars is responsible growth, which stands for improving the lives of citizens and consumers but not at the detriment of unethical business practices.
    • [00:26:51] People should be able to do 90% of their daily activities without needing devices, and NEC is working to make it a reality. For example, ten or twenty years from now, when you open your car or get coffee, you can use your face or another biometric to conduct the transaction without a physical wallet or phone.

    They Cover:

    • [00:04:24] The technological revolution we are experiencing creates an explosion of opportunities for healthcare development. New tools allow us to test millions of possibilities for a specific problem and find the best one. Cloud labs represent a concept of robotic laboratories that are in demand so anybody can access them and request a specific experiment, which allows them to multiply the ability to find better solutions way faster.
    • [00:08:33] Over the past half-century, most healthcare technology companies have been chasing the wrong goals at the time healthcare technology has become more amazing. The affordability and accessibility of those technologies have become worse. Healthcare technology investment across high-income countries is directed at drugs and devices explicitly designed to capture higher and higher prices rather than trying to address larger needs. A real trend in the industry represents the possibility of giving the patient autonomy, control, and self-determination.
    • [00:12:20] Given today's problems, we must pay attention to designing products that will work for everyone regardless of socioeconomic status. Augmented reality has the potential to improve diagnostics and enable more advanced remote care. Technology that will enable healthcare providers to get more robust information about the patients they are seeing remotely may be a game changer for ensuring better care. Device developers should be careful to develop products that can meet the needs of the majority of the population rather than the privileged few and advocate for funding streams that encourage the development of devices and solutions to help manage chronic conditions across the entire population.
    • [00:14:41] Designing with intent in healthcare requires recognizing some secular trends. Firstly, life expectancy is increasing. However, the healthy average life expectancy today is about sixty-three years. There is a large gap between the average life expectancy of about seventy-three years and the healthy average life expectancy of sixty-three. Secondly, non-communicable diseases have risen dramatically and account for seventy percent of mortality globally.
    • [00:16:38] Most non-communicable diseases are influenced by social determinants of health, sleep, diet, fitness, and stress. Most people can influence these social determinants, so healthcare technology requires a design shift toward the individual. While the use of technology will include better medical equipment, more targeted and customized drugs, gene-based therapy, and personalized medicine, the most massive shifts will happen by leveraging relatively simple tools like a mobile phone but will require a massive shift in thinking and in the design of health systems.
    • [00:21:35] COVID taught us that people don't always want to come into the doctor's office. Therefore, one segment that has a great chance of emerging is the AI-based Internet of Medical Things or IOMT. One of the problems in AI is collecting enough data. Over time, that issue will get resolved, and more people will collaborate to build these models and share information.
    • [00:23:57] Technological advancements in healthcare can take different directions, like virtual reality, blockchain, machine learning, genomics, proteomics, and digital therapeutics. The continued development of preventative medicine and advancements in personalized medicine are two key areas healthcare technology designers should focus on. We need to shift from what we currently have as a procedural reimbursement model and start charging for the value being added from a healthcare standpoint.
