This is Shannon's Streamer, a pattern from Rich McElligott of the Illinois Smallmouth Alliance, and it's a dandy! Designed to imitate a Yamamoto Senko or Shade Shape Worm, this is the fly fisher's equivalent of a dead-stick bait.
