Member Spotlight: Patricia M. Byers, MD, FACS Sep 18, 2024

This spotlight is on Dr. Patricia Byers, MD, FACS. She is a Professor and Section Chief in Surgical Nutrition and Metabolic Surgery at the Ryder Trauma Center/Jackson Memorial Hospital in Miami, FL. Dr. Byers became interested in nutrition while working in trauma and critical care, where she saw the impact of nutrition support on this patient population. She credits her career development to mentors like Drs. Stanley Dudrick, Bruce Bistrian, and Miguel Echenique. Dr. Byers also discusses the importance of a multidisciplinary team in the care of patients receiving nutrition support, and how all members of the team are able to learn and grow from each other. She highlights the minimal nutrition training that surgical residents receive and yet surgical patients can benefit the most from nutrition support. In her institution, she makes sure that her surgical residents are exposed to their nutrition support team and gain experience in writing orders for TPN. Dr. Byers is an active member of ASPEN and contributes to various committees, including the Physician Engagement Committee. She encourages others to get involved in ASPEN to promote their careers in nutrition. Member Spotlight: Jamie Bering, MD Aug 08, 2024

This podcast features Dr. Jamie Bering, MD, Senior Associate Consultant in the Division of Gastroenterology and Hepatology at the Mayo Clinic in Scottsdale, Arizona. Dr. Bering first became interested in nutrition as a GI fellow after she realized her specific interest in placing PEG tubes and working with those patients on their nutrition and hydration management post-procedure. She describes the importance that mentorship played in her career advancement and specialty in nutrition. She has been fortunate enough to have completed her training in the same institution, now working alongside her mentors. Dr. Bering describes gaps in the field of nutrition specific to conditions where the role of nutrition support has not yet been well defined. She is interested in researching these areas and developing an algorithm to best approach these patients and determine how nutrition support might improve their overall outcomes. Dr. Bering is a member of the Physician Engagement Committee and describes how her committee involvement and ASPEN membership overall has led to numerous advancement opportunities in her career. This video features Dr. Robert Martindale, MD, PhD, FASPEN, Professor of Surgery in the Division of Gastrointestinal and General Surgery, and Medical Director of Hospital Nutrition Services at the Oregon Health & Science University in Portland, Oregon. Dr. Martindale describes his fascinating career path. While working in a lab at UCLA during the summers to earn money for forestry school, Dr. Martindale became interested in human physiology. After completing his undergraduate studies, he was invited back to UCLA to work in the nutrition department as a graduate student. Here, he earned his PhD in Nutrition. This interest in nutrition then led him to enter medical school, specializing in surgery, all while enrolled in the Army which allowed him to pay for medical school. Dr. Martindale discusses several nutrition-related topics, including specialized pro-resolving mediators (SPMs), the use of prebiotics and probiotics, food and the microbiome, managing patients with fistulas, and the overall future of nutrition. This video features Dr. Nisha Mangalat, Associate Professor of Pediatrics in the Division of Gastroenterology at the St. Louis University School of Medicine and Medical Director for the Glennon Intestinal Rehabilitation and Feeding (GIRAF) Program at SSM Health Cardinal Glennon Children’s Hospital. The GIRAF program was formed around a multidisciplinary, collaborative approach to caring for complex pediatric patients with intestinal failure. The GIRAF team meets weekly to discuss patient cases and review updates provided by the patients’ parents/caregivers. This enables the team to make proactive, systematic adjustments to each patient’s nutrition therapy. Dr. Mangalat shares that the name “GIRAF” was coined by their registered dietitian while attending an ASPEN meeting. Dr. Mangalat holds nutrition education very close to her heart and notes several gaps in the current medical education curriculums. Dr. Mangalat is the director of the pediatric gastrointestinal fellowship program and offers some insight into how we can engage future physicians to get involved in nutrition. She advises young professionals to not be afraid to say “yes” when it relates to their career and to explore if they are curious. Whether an avenue has been explored or not, Dr. Mangalat encourages others to think, “Why not me?” when researching something they have questions about. Dr. Abdelhadi developed an interest in nutrition when she was asked to step in and develop a one-week nutrition curriculum for medical students. She was surprised to learn that there is no dedicated nutrition content in undergraduate and medical education. Dr. Abdelhadi shared two priorities. The first is to identify malnutrition in every clinical encounter and not miss out on opportunities to address it. The other is to provide nutrition education to all medical students, regardless of their area of specialty or education level. She emphasizes the importance of collaboration, noting that multidisciplinary work offers so much to not only patients but also professionals. Dr. Abdelhadi recommends that young professionals identify their passion and then find opportunities in all scholarly areas—teaching, clinical work, and service. This video features Dr. Ben D. Albert, Assistant Professor of Anaesthesia at Harvard Medical School and Associate in Critical Care Medicine and Pain Medicine at Boston Children’s Hospital. Dr. Albert credits the mentorship from ASPEN's past president Nilesh Mehta, MD for sparking his interest in nutrition during his time as a critical care fellow. He hopes to educate and inspire young physicians in this same way—through encouragement to participate in research and presentations and by inspiring with hands-on technology at the patient's bedside. As a researcher in pediatric critical care nutrition, Dr. Albert shares his experience in how a tiny fellow project can snowball into a larger project that maps out the next five to ten years of his career. He strongly encourages involvement in ASPEN and going to national meetings to get inspired and make connections. He joined the abstract review committee and research subcommittee when he was a junior faculty and has been involved with ASPEN ever since, now serving as a member of the physician engagement committee and the associate editor for JPEN. He describes the goal of involvement in ASPEN as being something that doesn’t feel like extra work because you enjoy doing it so much and encourages others to get involved. There, he recognized the tremendous impact of nutrition support on the recovery of trauma patients and their outcomes. Dr. Alverdy received the 2023 George Blackburn Research Mentorship Award. He recalls his own mentors such as Dr. Doug Wilmore and Dr. George Blackburn in the 80s and early 90s. Through these mentors, he was inspired to study nutrition as a key measure in patient care. Member Spotlight: Justine M. Turner, MD, PhD May 16, 2023

This video features Dr. Justine M. Turner, MD, PhD, Professor of Pediatrics and Divisional Director of Gastroenterology and Nutrition at the University of Alberta in Canada. Dr. Turner’s interest in nutrition stemmed from her original interest in Gastroenterology. Looking back, she felt there was a lack of nutrition education in her medical training, which she only realized once she began to pursue her PhD in nutrition. She notes there are too few physicians with PhDs in nutrition. Dr. Turner spends 40% of her time on research and 35% on clinical practice. Currently, she runs a translational animal lab looking at growth factors for adaptation, TPN lipids, and enteral nutrition in short bowel syndrome. In her clinical practice, Dr. Turner deals mostly with Celiac disease and gluten free diet while her inpatient service looks after solid organ and intestinal transplant patients. She highlights that nutrition is integrated in everything that she does. She also shares her insights on the differences between the delivery of healthcare in Australia and Canada and how each were affected during the Covid-19 pandemic. She insists that there is no better home for her interests than ASPEN. It has provided her with incredible opportunities to collaborate, present her data, and receive recognition for her work. ASPEN is also a reliable place for her students to learn and grow. She has served on the ASPEN research committee and currently serves as the Chair of the Publication Education Committee. Dr. Turner has three pieces of advice for junior faculty: find multiple mentors, be adaptable, and to say “yes” to be on committees or societies. Dr. Mercer sees nutrition as pivotal in every decision made at his intestinal rehabilitation program. As a transplant surgeon, the practice has evolved so much that he spends most of his time focusing on intestinal rehabilitation instead of transplant. Improvements in medical therapy, PN and lipids have helped patients with short intestinal length thrive and enabled more to come off PN. Dr. Mercer shares that ASPEN is where the intestinal failure thought leaders are, and the ASPEN conference is one that he never misses. He believes nutrition care is no longer multidisciplinary but transdisciplinary. Clinicians are no longer pigeonholed in their roles and can work across perceived roles, within the scope of practice. If a young clinician finds any of this interesting, he recommends calling any of the big centers and asking for opportunities. This video honors Dr. Praveen S. Goday, MBBS, Director of Feeding and Nutrition Programs at Nationwide Children’s Hospital in Columbus, Ohio. He is also a Clinical Professor of Pediatrics at The Ohio State University. Dr. Goday’s journey to nutrition was unplanned. He was recruited by his mentor to run the nutrition program in Milwaukee, despite having had no nutrition training. Dr. Corkins said he was fortunate in that nutrition was a focus in every step of his training. In medical school, his biochemistry professor said that students didn’t get enough nutrition and was given an hour each week to teach it. He later found out this focus on nutrition was not the norm in medical training. Dr. Corkins works as an academic physician and loves to teach nutrition. He says there are still huge holes in nutrition knowledge, despite significant progress in recent years. There are still lots of questions that need answers. This video honors Dr. Maitreyi Raman, Associate Professor of Medicine at the Foothills Medical Centre in Calgary, Alberta in Canada. She is also the Medical Director of the Southern Alberta Home Enteral and Parenteral Nutrition Program. After completing her nutrition fellowship around 2004, she noticed that there were very few nutrition-focused clinician/scientists within the gastroenterology landscape. She adds that her ability to find a female mentor in this field inspired and influenced her career path. Over the past 10 years, her research focus has shifted from enteral and parenteral nutrition to evaluating and accessing the role of dietary therapies, dietary components, and malnutrition in IBD. She encourages clinician/scientists to integrate nutrition into gastroenterology research reaching beyond enteral and parenteral nutrition to liver disease, IBD and beyond. Member Spotlight: Ajay Jain, MD, DNB, MNA Mar 24, 2022

This video honors Dr. Ajay Jain, Professor of Pediatrics, Pharmacology, and Physiology at St. Louis University, St. Louis, MO. His journey to nutrition began in medical school where he cared for babies needing parenteral nutrition (PN). While PN sustained them, many had liver injuries that progressed as they stayed on PN and needed liver transplants. This motivated Dr. Jain to focus on novel strategies to mitigate injuries and side effects of PN patients. He credits ASPEN with building his research niche. His first grant was from the ASPEN Rhoads Research Foundation. ASPEN’s resources, like Malnutrition Awareness Week, also helped him grow from a clinical perspective. Dr. Jain advises junior faculty to never give up on their ideas. He feels that today’s relative value unit (RVU) healthcare model does not give clinicians time to do research. This video honors Jose Pimiento, MD, FACS, Associate Professor of Surgery, in the division of Gastrointestinal Oncology, at the Moffitt Cancer Center in Tampa, FL. Dr. Pimiento had the honor of training with Dr. Stanley Dudrick, the father of intravenous feeding. He shares some of Dr. Dudrick’s philosophy that as a surgeon, you need to be capable of handling everything, including the nutrition of your patients. Dr. Dudrick was upset with the approach of waiting for the next day to feed patients— tomorrow the patient will not have tests and will eat better, and then tomorrow comes and you’re doing something else on the patient, and so on. He was surprised that every day, we would replace electrolytes, but never think about replacing calories and protein. Dr Pimiento also talks about his innovative research work in nutraceuticals and cancer chemoprevention. ASPEN Lipid Injectable Emulsion Safety Recommendations, Part 2: Neonate and Pediatric Considerations Nov 16, 2021

Lipid injectable emulsions (ILEs) are complex pharmaceutical formulations intended as a source of energy and fatty acids for parenteral nutrition (PN). Part 1 of this series addressed adult use. Part 2 addresses ILE safety in neonatal and pediatric patients, who often require higher doses compared with adults to support growth, development, and daily metabolic needs. ILE is often administered as a separate infusion owing to compatibility and stability issues and limitations of access. It is associated with PN errors in the administration, prescribing, and transcribing processes and has been associated with adverse effects, including effects from doses exceeding manufacturer recommendations. This paper describes safe use of ILE products for US neonatal and pediatric patients, including indications, prescribing, order review, preparation, administration, and monitoring. He has over 30 years of experience in nutrition support consulting, clinical research, and teaching. He’s currently the section editor for nutrition at UpToDate. Dr. Seres shares his thoughts on how the field of nutrition has changed, and not changed (lack of randomized control trial data), and what’s to come (personalized nutrition). He leads one of the few nutrition fellowship programs and points out that in the ICU, patients on average receive only 45% of the calories prescribed to them. During his early years of practice, Dr. Mehta realized there was very little evidence-based guidelines to drive nutrient delivery to sick children at the bedside. Depending on the team, the child was fed differently and the fundamental question of how much food/calories to deliver was unanswered. This began his journey into research. Dr. Mehta stresses that nutrition in the ICU is a team effort and that bedside nurses are his partners, the drivers of change. Member Spotlight: Robin Mendelsohn Jun 16, 2021

This Spotlight honors Robin Mendelsohn, MD, clinical director of Gastroenterology, Hepatology and Nutrition Services, and associate professor of clinical medicine at Memorial Sloan Kettering Cancer Center (MSK) in New York City. Dr. Mendelsohn’s interest in nutrition was solidify after she had 6 months of nutrition focus in her GI fellowship at MSK. Today, she runs the nutrition services at MSK and offers a dedicated nutrition fellowship. She notes that when she first started, obesity management was not a focus when treating cancer patients even though obesity is a risk factor for so many cancers. But now patients are living longer with cancer and obesity management is incorporated into the treatment. Dr. Mendelsohn is also co-director at the Center of Young Onset Colorectal Cancer where they provide coordinated care for patients under 50 years of age. She says many want to learn how nutrition can get them through treatment and survivorship. She’s doing research to see why colorectal cancer is increasing in younger people, it’s if caused by diet and/or environment. When Dr. Stapleton was a fellow, she became interested in research and saw nutrition in the ICU as an open box, with many unanswered questions. She has since led multiple nutrition RCTs, including a trial examining the effect of combined exercise and protein delivery in critically ill patients. Her advice to trainees with a research interest is to find good mentors, learn from the research coordinators, focus on developing strong writing skills and invest time to take workshops on grantsmanship and publishing. She works in Mayo’s home PN program where she and her multidisciplinary team provide long-term care to patients on PN. Dr. Bonnes’ one advice to PN clinicians is to reach out to team members and peers for help. She says ASPEN is part of her team. Because clinical nutrition touches every aspect of care, that it’s a natural process, he says it doesn’t get the attention it deserves. He feels that surgical residents have a genuine interest in clinical nutrition, an interest he didn’t feel his peers had when they were residents. He credits ASPEN and educators for sparking this interest. This podcast highlights an abstract from ASPEN20 in which a study looking at trends of liver profiles, micronutrients, nutrition, and growth for pediatric intestinal failure patients undergoing intestinal rehabilitation while using a mixed oil intravenous lipid emulsion with the goal of providing up to 30% of parenteral nutrition calories from lipids. The study was successful in delivering up to 30% of calories from lipids, allowing a decrease in glucose infusion rate and an increase in concentration of kcals per hour reducing overall hours per day of TPN. A Retrospective Evaluation of a 4-Oil Lipid Emulsion on Liver Function for Home PN Patients Aug 24, 2020

The study demonstrated that 4-oil lipid emulsions were a safe and effective alternative to SO emulsions for HPN patients.
